The HACC Program assists residents of the Woden Valley to retain
independent living within their own homes and with full access to
the community and its services.
Support provided includes assessment of individual needs, care
planning, incorporating transport, activities, assistance within
the community, information, referrals to other support agencies,
emotional support and/or general assistance.
All HACC programs are available to individuals and families who:
- Are elderly people with a disability
- Are younger people with a disability
- Are carers of both
HACC programs are funded by the Federal Department of Health
and Aging and ACT Health.
HOME AND COMMUNITY CARE (HACC) – provides services to both older people and those younger with a disability living in the Woden area. The aim of these HACC services is to assist people to live independently in their own home, if possible.
HOME AND COMMUNITY CARE (HACC) programs providing support include:-
- Community Transport: For those people unable to access public transport and need to attend medical appointments etc. a $3 fee applies.

- Volunteering: Home and Community Care (HACC) actively engages with our Volunteers to assist with HACC program activities, such as social groups, transport, shopping assistance etc. Phone Geoff: 6234 6862
Assistance with Care and
Housing for the Aged (ACHA)
The ACHA Program assists financially disadvantaged older persons
who are in rental accommodation or who are homeless.
The program aims to meet client's support and accommodation needs
in order for them to remain living within the community.
Assistance with Care and Housing for the Aged is funded by the
Commonwealth Department of Health and Aged Care.

New Woden Valley Community Bus Service
A low cost community bus, for people who are socially isolated and unable to use public transport, is now available in the Woden Valley. 
The 22 seater bus is one of a fleet of six that is being provided to regional community service organisations by ACTION with funding for a bus driver and transport coordination provided by the ACT Government’s Department of Disability Housing and Community Services.
One bus has been allocated to Woden Community Service and it will run Monday to Friday between 10am - 4pm.
Chris Flaherty, Manager of Home and Community Care Services at Woden Community Service, said the bus would be of great benefit to people in the Woden area who were house bound due to inability to take public transport, drive or who are reliant on others for transport.
"Community transport was one of the key services identified
at a Woden Valley Community Forum last year," Ms Flaherty said.
"Woden Community Service already provides transport to people
who are elderly, living with a disability or a mental illness.”
"This bus will complement that service as it will be able
to transport more people, particularly to activities that increase
their engagement in the community and maintain good health and well
being.”
"The bus service will not be an alternative for people not
wishing to use existing ACTION services. “People wishing to
access the service will be assessed for their eligibility in an
over the phone interview," ”Ms Flaherty said.
