The Transition to Recovery program (TRec) is an intensive community outreach mental health program to assist people to transition to the community following an acute episode of mental illness and to prevent relapse. 

How can a TRec Worker help?

The program is designed to help individuals maximize their potential for recovery and independence after being discharged from the hospital or for those who require additional support in the community to manage sub-acute symptoms and prevent hospitalisation.

This program runs for up to 12 weeks and offers intensive outreach support, including calls on evenings and weekends. 

Here is what the program can provide:

  • Outreach services within your community Monday through Friday from 9 AM to 5 PM, as well as access to phone support every evening until 9 PM, including weekends.
  • Assistance with linking to services, providing social support, and helping you attend medical appointments and community-based programs.
  • A space to discuss any concerns you may have about managing mental health symptoms and day-to-day living needs.
  • Psychosocial education to help you manage mental health symptoms, empowering you to engage in activities that you enjoy.

Who can access the program?

Individuals are eligible and can voluntarily access the program if they meet the following criteria:

  • are current consumers of Mental Health, Justice Health, and Alcohol & Drug Services (MHJHADS)
  • have multiple and simultaneous care needs, including psychosocial and clinical interventions and support.
  • have a referral from a public Mental Health Service provider – see details below.

How to access TRec

Referrals to the program are made by Access Mental Health Team or Community Recovery Services (CRS) in City, Belconnen, Gungahlin, Tuggeranong, and Woden.

A referral can also be made by one of the following Canberra Health Services teams, with endorsement of CRS or Access Mental Health Team:

  • Home Assessment and Acute Response Team (HAART)
  • Canberra Hospital Adult Mental Health Unit (AMHU)
  • Adult Mental Health Recovery Unit (AMHRU) at University of Canberra Hospital
  • Acacia Adult Mental Health Unit at North Canberra Hospital
